Who to contact
Eremia Therapy is a counselling practice located at 105–1465 Salisbury Ave, Port Coquitlam, BC V3B 3L4. For questions about privacy, requests to access or correct your personal information, or concerns about how information is handled, please contact Daniel Haddad, MS, CCC, at daniel.haddad@eremia.ca or (778) 557-5663.
Information you choose to share
When you contact Eremia by telephone or email, you may provide your name, contact details, scheduling preferences, and other information you choose to share. Please avoid sending sensitive clinical information by ordinary email. If you become a client, additional information is collected through the clinical intake and counselling process, subject to the practice's informed-consent and confidentiality procedures.
Appointment booking through Jane
Booking links on this site take you to Jane, a separate appointment and client-management service. Jane may ask you to sign in or create an account. Information entered on Jane's pages is handled through that service and Eremia's practice account. Review the notices presented within Jane before entering personal information. This website does not collect your Jane password or payment-card details.
Website technology and external services
The website is hosted through Netlify and may generate standard technical logs needed to operate and secure the site. Its design currently loads fonts from Google Fonts; visiting pages may therefore involve a request to Google's servers. Links to Google Maps and the Jane booking portal open third-party services with their own practices and policies.
Eremia does not currently use Google Analytics, Google Tag Manager, advertising pixels, or behavioural remarketing on this website. If optional analytics is added, this notice and any required consent choices must be updated before it goes live.
Why information is used
Information provided directly to Eremia may be used to respond to your questions, arrange appointments, deliver counselling services, handle administration and billing, maintain required records, and meet applicable legal and professional duties. Information is used and disclosed only as permitted or required in the relevant circumstances, including where you have provided consent or where the law requires or permits disclosure.
Safeguards, access, correction and retention
Personal and clinical information requires appropriate safeguards. Contact the practice if you would like to know more about the safeguards that apply to your information. You can ask about access to, or correction of, personal information held by the practice, subject to applicable legal limits. Retention and disposal of clinical records follow applicable professional and legal requirements; contact the practice for details about the rules that apply to your records.
Updates and questions
This policy may change if the website, service providers, or privacy practices change. The updated date will appear above. For questions about website privacy or confidential clinical communication options, contact the practice before sharing sensitive information electronically.
